* Medicare Plus fee level shows the cash payment level that you expect to receive. The basic starting point is that you will accept the MEDICARE fee schedule as your expected reimbursement level (indicated by "0" in this box to show 0% over Medicare). If you wish to charge more than the Medicare Fee schedule then indicate that in this box. For example if you wish to charge 5% over the Medicare Fee schedule enter "5" in the box. This file is an excel file that you can download with the Medicare Fee Schedule (2004). It also has a place to calculate the charge if you want to charge a percentage over the Medicare schedule.

Flat Fee means you will charge the given amount, in advance, to be inclusive of all services that are done in your office.

DOCTROIT will not share your information except for what shows up on the physician listing page. The email field is only for you to be able to retrieve your data in the future to make changes.

MD HUB's e-services mean that your patients go on-line to request appointments, prescription renewals, test results, referrals and to ask questions that then reach you via your fax machine. You might want to try it out yourself first.
